    Try the following, see if it helps.

    Wipe cache partition

    Turn off your device.

    Press and hold the Volume down and Power button at the same time.

    When the OnePlus logo appears, release the Power button, but continue holding the Volume down button.

    At the Password screen, enter the Password or PIN number that you use to unlock your device. When done, tap Ok.

    Tap English.

    Tap Wipe data and cache.

    Tap Wipe cache.

    Tap Yes to continue.

    Tap Reboot to restart the phone.

    If it works or not come back and let us know.

    When are you getting this error, is it in a particular app?

    What is the exact error as you see it on the screen.?

    I face this error mainly when i switch on my phone. It shows a message that "requesting handset configuration" followed by "phone operation failed, please try again".
    Along with this I am facing frequent network loss (zero network) and call drops.

    Requesting Handset configuration is a message to tell you that when you switch your phone on, it's looking to update APN settings. It's possible some part of your APN settings are not configured correctly.

    Check with you Network provider that your APN settings match theirs.

    The Phone operation Failed is due to the APN settings failing.

    Also reset your Mobile Network adapter to see if it can help.

    Settings, System, Reset Options, Reset WiFi Mobile Bluetooth.
    Restart the phone.

    If this does not fix it, it's possible their could be a Sim issue, you may need a new Sim card.

    I have already changed my sim card, replaced it with a new one and this works perfectly in another device.
    I also reset the network settings as per the customer care executive suggested following these same steps, but yet to no avail.
    I will check about the APN settings with my network provider.
